Health and Insurance Information

 

Welcome to Wabash College Athletics. I hope your summer is off to a good start. Please do not procrastinate on getting your insurance and emergency contact information into us.  

Wabash College uses a secure webpage to improve the efficiency of this process.  All Wabash College students have an account. Please go to the CSMI SportsWare website.

The Student Health Center and the Athletic Training Staff use this platform. Student Athletes are required to fill out the forms. Please go to the links provided below if you have any other issues when completing this process. 

Directions:

Please remember when logging into the CSMI SportsWare website you will be using your Wabash email account.

  • Click reset password. (You do NOT need to click Join Sportsware.)
  • Check your Wabash College email to create password.
  • Please go to the Wabash Health Insurance Information webpage for detailed instructions.

Complete all tabs and sign and submit your forms completely.  All forms are now fillable PDFs on the CSMI SportsWare website. Do not forget to check the signature box and the save and submit button when completed. You will also upload the front and back of your insurance card on the bottom of the insurance tab. The web page will also post your progress of completion when you close out. 

For Upperclassmen:

  1. Update your CSMI SportsWare account, as needed. Please Note: You must update all fields in the Insurance tab every year.
  2. Scan and upload the FRONT and BACK of your primary insurance card. 
  3. Insurance acknowledgment form, which must be signed by you and your parent or guardian. (Fillable PDF on the CSMI SportsWare website.)
  4. ADHD medication reporting form with testing scores (only If taking medication)
  5. Physicals take place on campus when your coach posts the reporting date.

For Freshmen and Others New to the Team:

1.     Create a password to log in to swol123.net and complete all tabs. An account has already been created for you. Just change your password. Complete all forms on the CSMI SportsWare website, all of which are fillable PDFs.  

2.     Scan and upload the FRONT and BACK of your primary insurance card. 

3.     Insurance acknowledgment form, which must be signed by you and your parent or guardian.

4.     Sickle cell form. (You must first watch the video linked on the Wabash Athletic Training webpage.)

5.     Concussion agreement form. (You must first watch the video on the Wabash Athletic Training webpage.)

6.     Review and sign the HIPPA form.

7.     Compete the NCAA compliance Registration instructions

8.     ADHD Medication reporting form (only If taking medication).

9.     Physicals will take place on campus when your coach posts the reporting date.

10.  An impact test will take place on campus when your coach posts the reporting date.

11.  Submit your Student Health Record and vaccination dates. The form is provided on the New Student Road Map.

In order to participate in intercollegiate athletics at Wabash College, you must have a primary insurance plan. The deadline for submitting proof of insurance is July 15, 2018. Your insurance is being sent to a verification agency to check the accuracy, which will take a week or longer to complete.  You are restricted to participate until your insurance is verified.

Your personal insurance plan will be billed as Primary. Wabash College does have a secondary plan for official practice and competitions. This plan does have a deductible.

Please remember that the NCAA insurance policy states that your primary insurance must cover you state-to-state with a minimum of $90,000. Please make sure your insurance plan covers you for intercollegiate injuries (some plans do not). Please check if your coverage is in network with medical providers in Crawfordsville and the Indianapolis area. 

Medicaid plan will not work; you must purchase a new plan.

HMOs will only cover you in your current living area. You will have to change your coverage area.  Full-time students are allowed to do this.
